    These are the only record cards I get on with. They were the ones I used after my SMAE training 20 years ago and find they contain all the information needed. The only criticism I have is that there is not enough space for the long lists of medication people often have these days. I'd also like to see a tick box/date issued for 'GDPR Issued' to comply with current data protection laws.

    Easy to fill out and sufficient info recorded for future use. However, on the back of the card I would like to see an arrangement to record medicines of patient as most of mine are on at least 6.
